- प्रवेश परीक्षा द्वारा चयन के लिए न्यूनतम अर्हकारी अंक 40 प्रतिशत का प्रावधान रखा गया है।
- समान अंक होने की स्थिति में जिस अभ्यर्थी की उम्र ज्यादा होगी वह मेरिट में दूसरे अभ्यर्थी से ऊपर होगा। समान उम्र होने की स्थिति में जिस अभ्यर्थी का कार्यानुभव ज्यादा होगा वह मेरिट में दूसरे अभ्यर्थियों से ऊपर होगा।
- प्रश्न पत्र में नर्सिंग के समस्त विषयों से सम्बंधित 100 MCQ का एक प्रश्न पत्र होगा। प्रत्येक प्रश्न में उत्तर के 4 विकल्प होंगे। प्रत्येक प्रश्न 1 अंक का होगा। नेगेटिव मार्किंग नहीं होगी। प्रश्न पत्र की समय अवधि 2 घंटे की होगी।
- प्रवेश परीक्षा में न्यूनतम 40 प्रतिशत अंक अर्जित करने परीक्षार्थियों का कौशल एवं दक्षता आंकलन (Skill & Competency Assessment) किया जावेगा। कौशल एवं दक्षता आंकलन में उत्तीर्ण होने पर ही परीक्षार्थी सर्टीफिकेटे कोर्स इन कम्युनिटी हेल्थ में प्रवेश हेतु पात्र होंगे। ऑनलाइन प्रवेश परीक्षा एवं कौशल एवं दक्षता आंकलन के आधार पर मेरिट सूचि जारी की जाएगी।
- कौशल एवं दक्षता आंकलन (Skill & Competency Assessment) हेतु 2 स्किल्स का आंकलन किया जायेगा। प्रत्येक स्किल के लिए 20 अंक निर्धारित होंगे। कुल 40 अंकों में से उत्तीर्ण होने के लिए 32 (80%) प्राप्त करना अनिवार्य होगा।
- लिखित तथा दक्षता आंकलन प्रक्रिया के कुल मार्क के आधार पर अंतिम मेरिट लिस्ट जारी की जाएगी।

Candidates must compulsorily carry the following basic certificate in the program study allocated at the time of admission in the Certificate Course in Community Health Training-
- B.Sc. Nursing Verified Photocopy of Passed Marks
- 10th Class Marksheet Verified Photocopy.
- Live Registration Certificate of Registrar Nurses Counseling
- Verified photo of new passport size
- Annexure amount of Rs 2 lakh on stamp paper of Rupees 100, in which the eligible candidate at the Health and Wellness Center should mention the consent of continuous minimum three year job acquisition.
- Subdivision Officer, Revenue, Madhya Pradesh, original copy of certificate issued and verified photocopy.
